Hospice care is for people with a life-limiting illness, who have six months or less to live in the estimation of a physician. Patients often come to us with a diagnosis of:

How Does Hospice Work?


Once the patient is referred to hospice, we will schedule an informational meeting with the patient and family to discuss hospice care services. If the patient makes the decision to elect hospice care, the nurse will assess the patient and contact the hospice medical director to determine if the patient is eligible for hospice care.


Step 1: The medical director determines the patient is eligible for hospice care with a terminal illness and prognosis of six months or less to live if the disease takes its normal course.


Step 2: In collaboration with the patient, caregivers and their doctor, the hospice team develops a plan of care based on the patient and family’s needs. Hospice services may include:

  • Nursing care, such as symptom and pain management, education for patients and families, and wound care
  • Medical equipment, supplies and medications related to the hospice diagnosis
  • Help with daily tasks like bathing, dressing and grooming from a hospice aide, if needed
  • Social work services to provide emotional support and help with end-of-life planning
  • Physical, occupational and speech therapy if needed
  • Dietary counseling if needed
  • Spiritual support with a chaplain
  • Bereavement counseling to help family members process grief and loss


Step 3: The patient receives regular visits from the hospice care team. As their needs change over time, their plan of care adjusts as well.
